چکیده

Abstract Background Guideline-directed medical therapy (GDMT) optimization is mandatory before transcatheter edge-to-edge mitral valve repair (M-TEER) in patients with secondary regurgitation (SMR) and heart failure reduced ejection fraction (HFrEF). However, the effect of M-TEER on GDMT unknown. Objective To evaluate frequency prognostic implications predictors after SMR HFrEF. Methods This a retrospective analysis prospectively collected data from EuroSMR Registry. The primary events were all-cause death composite or HF hospitalization. Results Among 1641 patients, 810 had full datasets regarding included this study. occurred 307 (38%) M-TEER. Proportion receiving ACEi/ARB/ARNI, betablockers MRA was 78%, 89% 62% 84%, 91% 66% 6-month (All p<0.001). Patients lower risk (adjusted hazard ratio [HR] 0.62; 95% confidence interval [CI] 0.41-0.93; p=0.020) hospitalization HR 0.54; CI 0.38-0.76; p<0.001) compared those without. Degree MR reduction between baseline follow-up an independent predictor OR 1.71; 1.08-2.71; p=0.022). Conclusions HFrEF considerable proportion independently associated rates for mortality hospitalizations. A greater decrease increased likelihood optimization.

Acute Mitral Regurgitation Secondary to Papillary Muscle Tear: Is Transcatheter Edge-to-Edge Mitral Valve Repair a New Paradigm?

Severe mitral regurgitation (MR) secondary to papillary muscle tear is an infrequent but overwhelmingly morbid and often fatal complication of acute myocardial infarction occurring in 1% to 3% of patients. In-hospital mortality without surgical correction can approach 80%; however, even with surgical correction, papillary muscle tear carries a mortality rate between 19% and 53%. While considera...

Endovascular Edge-to-Edge Mitral Valve Repair

Background—The edge-to-edge technique is an accepted method for the surgical repair of a regurgitant mitral valve. This study reports the initial use of an endovascular technology that enables a double-orifice edge-to-edge mitral valve repair without cardiopulmonary bypass in an animal model. Methods and Results—Adult pigs (n 14) were anesthetized, and left thoracotomy was performed for epicard...

Edge-to-edge mitral valve repair without ring annuloplasty for acute ischemic mitral regurgitation.

BACKGROUND Alfieri edge-to-edge mitral repair has been used clinically with ring annuloplasty to correct ischemic mitral regurgitation (IMR), but its efficacy without concomitant ring annuloplasty has not been described in this setting. METHODS Seventeen sheep underwent implantation of 9 radiopaque markers on the left ventricle, 8 on the mitral annulus (MA), 1 on each papillary muscle (PM) ti...

percutaneous mitral valve repair with the edge-to-edge technique: case series of first iranian experience

mitral regurgitation (mr) is a common valvular lesion in the general population with considerable impact on mortality and morbidity. the mitraclip system (abbot laboratories, abbot park, il, usa) is a novel percutaneous approach for treating mr which involves mechanical edge-to-edge coaptation of the mitral leaflets. we present our initial experience with the mitraclip system in 5 patients. in ...

Mitral valve repair for functional mitral regurgitation in end-stage dilated cardiomyopathy: role of the "edge-to-edge" technique.

BACKGROUND The aim of this study was to assess the results of mitral valve (MV) repair in functional mitral regurgitation because of end-stage dilated cardiomyopathy (DCM). METHODS AND RESULTS Seventy-seven patients with end-stage idiopathic (26 patients) or ischemic (51 patients) DCM underwent MV repair for functional mitral regurgitation (3 to 4+/4+). Fifty-eight patients (75.3%) were in Ne...
